Registration Forms

All riders who are new to Pegasus Therapeutic Riding Academy must fill out the Registration and Medical forms. In addition, all existing riders must fill out the Annual Rider Medical Update Form on an annual basis.

Able-bodied adults who would like to register for our Back in Tack program should download and fill out the forms below.

The Pegasus TRA 4-H club is open to all school age Pegasus riders, siblings and children in the Philadelphia and surrounding community. For more information on this inclusive program, please email Ryan@PegasusPhilly.org
